This form is used to apply for an INS travel document, reentry permit, refugee travel document, or advance parole.

The INS office to file this form varies according to what is being requested. Please be sure you know what you are requesting - reentry permit, refugee travel document, advance parole or humanitarian parole - before submitting this form to INS.
